Patient Portal

The Patient Portal is a link between patients and their doctor. Equipped with a secure username and password, you get anytime access to your Medical Health Records from the comfort of your home or office 24x7. Portal access is the fastest and easiest way to:

• Complete your Medical History and other office forms

• Request an appointment or prescription refill

• Check status of your appointments

• View Messages sent by the Clinic

• View your office notes and other records.

When you first register with the office, you will receive a welcome email.
Your user ID is your email address, Your temporary password is your first and last initial followed by your date of birth as an 8 digit number with no slashes, hyphens, or other punctuation.
For example, if your name was John Doe and birthday was July 4 1980, your password would be jd07041980

The system will prompt you to create your own unique password, then restart the login process using the password you created. The new password must be between 8 to 10 characters long and include at least one letter and at least one number. All letters are case-sensitive.

You will then be guided through completing your registration. We have already scheduled your first appointment, you can skip the Request Appointment step during the registration process.
